7ABSTRACT Stable coronary artery disease is influenced by several well-established risk factors suck as obesity, serum lipids, diabetes, and blood pressure. It has been well studied the role of serum total cholesterol and serum LDL and HDL cholesterol in predicting the risk of developing coronary heart disease. More recently, increasing evidence has suggested that serum triglycerides (TG) may be an independent risk factor for Coronary artery disease, leading to identifying individuals more likely to experience a coronary event. Myocardial ischaemia provoke mitochondrial metabolic dysfunction due to reduced oxygen delivery to the tissue, resulting in a decreased ATP generation by oxidative phosphorylation. The main determinants of the rate of fatty acids oxidation by heart are the concentration of free fatty acids (FFA) and the activity of key mitochondrial enzymes involved in fatty acyl transport and β-oxidation. Carnitine has vital role in the transport of long-chain fatty acid into mitochondrial matrix. In this study, serum lipid profiles and serum carnitine levels were measured in fifty two healthy subjects and fifty two stable angina patients. Blood samples were obtained after an over night fast from both groups. Serum total cholesterol, triglycerides and HDL-cholesterol were measured using standard enzymatic procedures whereas LDL was calculated 8depending on the Friedewald equation. Serum carnitine levels were determined using enzymatic colorimetric method. The results showed that serum concentration of TG was significantly low in male controls as compared to male patients (P < 0.001). However, serum concentration of TG was slightly low but not significant in female controls as compared to female patients. Furthermore, there was no significant difference in serum concentration of cholesterol and HDL between male and female control subjects as compared to patient groups. Serum LDL concentration was significantly low (P < 0.001) in all control subjects when compared to patients. On the other hand serum concentration of free carnitine (FC) was significantly low (P < 0.01) in male controls as compared to male patients. However, there was no significant difference in serum concentration of FC in female controls as compared to female patients. Furthermore serum concentration of short acylcarnitine (SC) was significantly low (P < 0.001) in male and female controls as compared to patients. Serum concentration of long chain acylcarnitine (LC) showed significant increase (P < 0.001) in male healthy controls compared to male patients. However, serum concentration of LC was significantly low (P < 0.001) in female controls as compared to female patients. The concentration of acylcarnitine (AC) was 9not significantly different in male controls as compared to male patients. Whereas, serum concentration of AC was significantly low (P < 0.001) in female controls compared to female patients. Serum concentration of total carnitine (TC) was similar in male healthy controls and patients. However, serum concentration of TC showed substantial increase (P < 0.01) in female patients than control group. In conclusion the results showed that stable angina caused significant change in serum levels of carnitine as compared with control subjects. The results also indicated that stable angina was associated with high serum levels of FC and SC in male patients. However, stable angina caused high serum levels of SC, LC, AC and TC in female patients as compared with healthy subjects. Changes in serum carnitine levels may result from alteration in lipid metabolism, with altered fatty acid transport into the cardiac mitochondria. Gender-related changes in serum levels of carnitine were also observed between control and patient groups. Therefore, it can be speculated that sex hormones and their precursors may be involved in changes of carnitine metabolism.
